Welireg (Belzutifan): Transforming RCC Treatment with HIF-2α Inhibition

Belzutifan, marketed as Welireg, is a groundbreaking selective inhibitor of hypoxia-inducible factor 2 alpha (HIF-2 alpha). Initially developed by Peloton Therapeutics, it was later acquired by Merck in 2019. This innovative therapy received approval in August 2021 for treating advanced renal cell carcinoma (RCC) in patients who had previously undergone treatment with programmed death receptor-1 (PD-1) or programmed death-ligand 1 (PD-L1) inhibitors, as well as vascular endothelial growth factor tyrosine kinase inhibitors (VEGF-TKIs).


The drug’s efficacy was demonstrated in the LITESPARK-005 clinical trial, which focused on patients with unresectable locally advanced or metastatic clear cell RCC that had progressed despite prior treatments. This approval marks a significant advancement in the treatment options available for RCC patients who have exhausted other therapies, offering a new mechanism of action in the fight against this challenging form of cancer.
