One of the more prominent trispecific antibody candidates currently in development is Restoretandtrade; (MK-3000, formerly EYE103), developed by EyeBio, which has since been acquired by Merck. Restoretandtrade; targets the FZD4, LRP5, and LRP5 proteins and acts as an agonist of the Wingless-related integration site (Wnt) signaling pathway. In September 2024, Merck initiated the Phase 2b/3 BRUNELLO trial for the treatment of diabetic macular edema (DME). The trial follows promising results from the Phase 1/2 AMARONE study, which explored MK-3000's efficacy in DME and neovascular age-related macular degeneration (NVAMD). Trispecific antibodies are now being explored for a range of diseases, including microbial infections, autoimmune conditions, and even rare diseases. A notable example is research from the Wistar Institute, where a trispecific T-cell engager (DTriTE) was developed to target EGFRvIII and IL-13Randalpha;2 in glioblastoma. Among the three DTriTE constructs tested, DT2035 demonstrated the strongest preclinical efficacy. In laboratory tests, DT2035 activated T cells, stimulating the production of key cancer-fighting cytokines, such as interferon-gamma (IFN-andgamma;), tumor necrosis factor-alpha (TNF-andalpha;), and interleukin-2 (IL-2), indicating significant potential for solid tumor immunotherapy.

 

The market for trispecific antibodies is also seeing increased involvement from both large and small companies. Big pharma companies like Sanofi and BeiGene, as well as specialized firms such as Mabworks, are actively engaged in the development of trispecific antibodies, utilizing proprietary platforms to enhance the antibodies’ effectiveness. Both are designed to target specific tumor antigens and enhance immune activation, showing promising therapeutic potential for solid tumors.

 

Startups are also playing a key role in advancing trispecific antibody development. A prime example is Oblenio Bio, a new company formed in November 2024 through a collaboration between Aditum Bio and Leads Biolabs. Oblenio Bio is focused on advancing a first-in-class trispecific T-cell engager antibody for autoimmune disorders. The company has secured an exclusive option to license LBL-051, a trispecific antibody targeting CD19, BCMA, and CD3. This innovative antibody is designed to deplete a broader range of pathological B-cell populations, making it applicable across a wide spectrum of antibody-mediated autoimmune diseases. The emergence of startups like Oblenio Bio reflects the growing interest and investment in trispecific antibody therapies, particularly for conditions beyond cancer.

 

As the trispecific antibody market continues to evolve, it is clear that these therapeutic agents hold immense potential. With ongoing clinical trials and the development of novel technologies, trispecific antibodies could soon revolutionize treatments for a variety of diseases, offering more precise, potent therapies for conditions that have long been challenging to treat. The combination of large pharmaceutical companies and innovative startups working in collaboration or independently underscores the exciting future of trispecific antibodies as a key area of biomedical research.
