A166, developed by KLUS Pharma, is a third-generation antibody-drug conjugate (ADC) designed to target HER2-positive cancers. It consists of a humanized anti-HER2 antibody, identical to trastuzumab, conjugated to a potent auristatin-based payload called Duostatin-5 (Duo-5), an anti-microtubule agent. This conjugation occurs via a stable, cleavable valine-citrulline linker, which ensures the cytotoxic agent is released inside cancer cells, triggered by the upregulated lysosomal cathepsins in the tumor microenvironment. This targeted release mechanism minimizes premature toxin release, reducing systemic toxicity.

Currently, A166 is undergoing Phase 2 clinical trials in China, primarily for the treatment of breast cancer. The trials aim to assess the drug's efficacy and safety profile compared to existing therapies, with early preclinical data indicating that A166 may provide superior tumor growth inhibition over other HER2-targeted therapies like T-DM1, positioning it as a promising option for patients with HER2-positive breast cancer.
