It consists of a humanized anti-HER2 antibody, identical to trastuzumab, conjugated to a potent auristatin-based payload called Duostatin-5 (Duo-5), an anti-microtubule agent. This conjugation occurs via a stable, cleavable valine-citrulline linker, which ensures the cytotoxic agent is released inside cancer cells, triggered by the upregulated lysosomal cathepsins in the tumor microenvironment. This targeted release mechanism minimizes premature toxin release, reducing systemic toxicity.
